Learning from reportable adverse events and near misses is an indicator of a proactive Culture of Excellence SM

Evidence suggests that as few as 10% of healthcare near-miss, patient safety-related events are reported through formal reporting channels. Through the implementation of a team debriefing system in a high volume multi-specialty U.S. operating room, frontline providers and staff were able to capture and record over 300 potential teamwork and system anomalies in the first four months of the implementation. These “glitches” revealed early indicators or ‘good catches’ of potential error and were reported at a factor of 12 times higher than the established reporting rate. Team satisfaction increased by 16% during the debrief intervention.
