Medication Safety: Expect the Unexpected

The North Carolina-Virginia Hospital Engagement Network (NoCVA HEN) is a group of 117 hospitals in North Carolina and Virginia working to meet the goals of  Partnership for Patients, a national initiative which aims to reduce patient harm by 40% and reduce readmissions by 20%. Hospitals who participate in NoCVA identify strategies to reduce harmful patient events, share solutions, and have access to learning opportunities.

As part of this initiative, participating healthcare facilities-the NC Center for Hospital Quality and Patient Safety, Healthcare Team Training, the Perinatal Quality Collaborative of North Carolina, and the Virginia Hospital and Healthcare Association are working together on a twelve month, regional collaborative designed to improve perinatal safety. Hospital teams will develop an actionable OB simulation training improvement plan integrating evidence-based teamwork and communication methods and strategies into OB clinical skills simulations.

HTT attended the Georgia Association for Nursing Education (GANE)2012 Annual  Conference at Callaway Gardens last week (February 16th-18th). It was a pleasure to meet Nursing Educators throughout the state of Georgia. Thanks to Everyone who stopped by our booth.

HTT President and CEO Steve Powell returned to Kameda Medical Center in Kamogawa City, Japan in January as part of a multi-year TeamSTEPPS® improvement initiative in Japan.
  Kameda Medical Center is a 925 bed hospital located on the Chiba peninsula.  Kameda is one of only two hospitals in Japan that are accredited by the Joint Commission International (JCI).  The goals of the initiative are improved teamwork and communication among staff and patients. HTT has implemented team-based care improvement initiatives in 18 countries outside the U.S. through our work with military and civilian healthcare organizations.

Understanding the depth and breadth of preventable patient harm within the African care-delivery system may be the first step to establishing sufficient urgency for change.

Patient safety has received increased attention in African countries in the last decade, yet little is known about African patient safety challenges and quality improvement opportunities. Most of the patient safety and quality improvement efforts have been made at the international level, particularly by the World Health Organization (WHO).

    Steve Powell, HTT President and CEO,  presented Human Factors Engineering Fundamentals training to the Perinatal Improvement Collaborative for 10 participating hospitals from Catholic Health Partners  (CHP). Over sixty clinical leaders of quality and patient safety improvement attended the workshop in Cincinnati, OH.  The course focused on standardization and reengineering processes using a systems approach to safe practice.  CHP has been an HTT client since 2007.

Over 60 Quality Improvement and Patient Safety leaders from across Nigeria participated in the 1st Educational Workshop on Patient Safety Culture sponsored by the Society for Quality Healthcare in Nigeria (SQHN).  HTT President and CEO Steve Powell facilitated the interactive workshop in Lagos, Nigeria on March 15th.  Representatives from across the country representing diverse public and private healthcare organizations participated in the educational sessions. Topics ranged from root cause analysis to measuring, analyzing and implementing safe practices and processes.  Established in 2006, the mission of the SQHN is to lead, advocate and facilitate the continuous improvement of quality and safety in healthcare in Nigeria through Education, Collaboration, Training, and Accreditation.
